- Fill the online application form carefully
- Upload Class 10th / Matriculation passing certificate
- Upload 10+2 / Intermediate marksheet with Physics, Chemistry, Biology & English with minimum 50% marks in aggregate and 50% in English
- Or upload marksheets of two-year vocational course with same subjects and same minimum marks requirement
- If applicable, upload Diploma/B.Sc in Pharmacy marksheet with minimum 50% marks in aggregate
- Upload a recent passport-size colour photo (taken after June 2025)
- Size: 10 KB to 100 KB
- Light background, front-facing, no facemask or headgear (except Sikhs)
- Candidate must hold a black slate with name and date written in white chalk
- Upload left-hand thumb impression image (10 KB to 100 KB)
- Upload candidate’s signature image (10 KB to 100 KB)
